1) Collect Registration form in Cork Deaf Association off – 5 McCurtain St

Email: mail@corkdeaf.ie to request a SouthDoc Registration form.
2) Fill out your information and either post or email the completed form to SouthDoc. Postal address can be found on the form or email it to reception@southdoc.ie
3) Wait for them to text you with the mobile number to use
4) If you need to book an appointment with SouthDoc, please include your name, date of birth, your location and your symptoms
